Marcadores bioquímicos del estrés en pacientes femeninas con reacciones a estrés grave y transtornos de adaptación, con referencia a la enfermedad cardiovascular: anti-hsp60 y LDL-Oxidada

Abstract
La mujer en sus diferentes roles está sujeta a exigencias que la hacen
propensa al estrés al desenvolverse en ambientes de estresores laborales,
familiares y personales. Con el objetivo de analizar la relación entre la escala
de Holmes y Rahe (EHR), el índice de reactividad al estrés (IRE-32) y los
marcadores bioquímicos de estrés como potenciadores de enfermedad
cardiovascular en pacientes femeninas con reacciones a estrés grave y
trastornos de adaptación; se realizó una investigación cuantitativa de caso–
control, en la que se estudiaron pacientes diagnosticadas en la unidad de
psiquiatría del IAHULA; mujeres con valores en la EHR > 250 y un grupo
control. Se midió el IRE-32, la tensión arterial, la circunferencia de la cintura;
los anticuerpos anti Hsp-60, la LDL-oxidada, la glicemia, el perfil lipídico y el
índice de Castelli. La prueba de Tukey arrojó diferencias significativas entre
los grupos de estudio para: Holmes y Rahe, IRE, LDL-oxidada, anti Hsp-60 y
el índice de Castelli. La EHR presentó relación lineal positiva con el IRE
global P(0,001), con la LDL-oxidada P(0,016) y con el índice de Castelli
P(0,003). La proteína anti-Hsp60 presentó una relación lineal directa con el
índice de Castelli P(0,005). Se concluye que a mayor estrés cotidiano mayor
índice de reactividad al estrés y mayor riesgo aterogénico por el consecuente
aumento de la LDL oxidada, el índice de Castelli y los anti Hsp60 y que si el
estrés es crónico, como es el caso de las pacientes con reacciones a estrés
grave y trastornos de adaptación, este riesgo es potencialmente mayor. Se
recomienda medir la Escala de Holmes y Rahe, IRE-32, LDL-oxidada, anti
Hsp60 e índice de Castelli en todas las pacientes con Women in their different roles are subject to requirements that make them
stress-prone, due to the fact that they are surrounded by environments with
different stressors elements (work, family and personal life). A research was
conducted in order to analyze the relationship between Holmes and Rahe
(EHR) scale, the reactivity to stress (IRE-32) index and biochemical markers
of stress as enhancers of cardiovascular disease in female patients with
reactions to severe stress and adjustment disorders. Through a quantitative
analysis of case-control, we studied patients diagnosed in the Psychiatric Unit
at the Hospital of The University of Los Andes (IAHULA). Subjects were both,
women with values in the EHR > 250, and a control group. The IRE-32, blood
pressure, waist circumference, Hsp-60 antibodies, the oxidized-LDL,
glycemia, lipid profile and Castelli index were measured. Tukey's test showed
significant differences between study groups in the following measures:
Holmes and Rahe, IRE, oxidized-LDL, anti Hsp-60 and the index of Castelli.
In addition, the EHR presented a positive linear relation with the global IRE P
(0.001), with the oxidized-LDL (0,016) P and P Castelli (0.003) index and the
protein anti-Hsp60 presented a direct linear relation with the index of Castelli
P (0.005). It is concluded that increased daily stress leads to higher rate of
reactivity to stress and more atherogenic risk due to the increase of oxidized
LDL, the index of Castelli and the anti-Hsp60. Moreover, if the stress is
chronic, as in the case of patients with reactions to severe stress and
adjustment disorders, this risk is potentially higher. It’s reccomended to
measure the EHR, IRE-32, oxidized-LDL, anti-Hsp60 and Catelli index in all
the patients with acute and chronic stress as cardiovascular risk enhancers.
