Abstract
Introducción. El crecimiento de la población envejecida ha provocado el aumento de la Enfermedad Renal Crónica. La progresión de esta enfermedad ha conducido a la necesidad de dirigir Cuidados Paliativos (CP) a los enfermos renales crónicos en estadio 5 en las diferentes opciones de tratamiento.
Objetivo. Revisar la literatura en relación a los CP y de soporte renal que se realizan en el área de nefrología en pacientes con Enfermedad Renal Crónica Avanzada (ERCA) al final de la vida en el ámbito hospitalario
Material y Método. Se realizó una revisión narrativa entre febrero-abril de 2016 en las bases de datos: Medline, Cinahl, Cuiden, Dialnet, Biblioteca Virtual de la Salud, Scielo y Google. Los criterios de inclusión fueron el idioma (castellano, portugués e inglés), período de publicación (2008-abril de 2016); teniendo que estar relacionados con los CP en pacientes con ERCA al final de la vida.
Resultados. Se revisaron 19 documentos. Se destaca la necesidad de coordinación entre Servicios de Nefrología y Unidades de Cuidados Paliativos desde fases precoces; la orientación de los cuidados al control de síntomas físicos, psicológicos y espirituales al final de la vida; y la importancia de la formación de las enfermeras y resto del equipo sanitario en CP en el proceso de muerte. Se recomienda el estudio de estas áreas debido a la limitada bibliografía para la práctica basada en la evidencia.
Conclusiones. Los CP han cobrado relevancia en el área de nefrología. En el final de la vida se precisa la atención holística e individualizada del paciente y su familia, además de una correcta formación de enfermeras en éste ámbito. Introduction. The growth of the aging population has led to the increase of Chronic Kidney Disease (CKD). The progression of this disease has led to the need to lead palliative care stage 5 Chronic Kidney Patients in different options of treatment.
Purpose. Revise the literature in relation with the Palliative Care (PC) and the renal support that are performed in the area of Nephrology in patients with Chronic Kidney Disease (CKD) at the end of life in the hospital environment.
Methods. It has been done a narrative review between February-April 2016 by consulting various databases: Medline, Cinahl, Cuiden, Dialnet, Biblioteca Virtual de la Salud, Scielo and Google Scholar. The articles chosen were in Spanish, Portuguese and English that were published between the year 2008 and 2016 and they had to be linked to PC in patients with CKD at the end of life.
Findings. A total of 19 documents have been included in this review. It is stood out above all the need for a coordination of the Nephrology Services and Units of Palliative Care since the early stages; the treatment orientation of the care is to control the physical symptoms, disturbance psychological and spiritual at the end of life; and the importance of training of nurses and the rest of team health on PC at the end of life. It is recommended the study of these areas because of the limited literature to carry out a comprehensive evidence-based practice.
Conclusions. The PC have gained relevance in the area of Nephrology. At the end of life is required a holistic and individualized care for the patient and his family, as well as a proper training of nurses in this field.
