Abstract
Introducción: los traumatismos constituyen un 12% de la carga mundial de enfermedad,
siendo, en el paciente politraumatizado, mayoritariamente frecuentes a nivel cervical
(38%), y asociados en muchos casos a una lesión medular aguda. Se estima que entorno
al 25% de los daños existentes en pacientes con lesión medular pueden ocurrir o verse
aumentados durante el manejo prehospitalario. Por ello, el objetivo de este trabajo es
analizar aquellos procedimientos indicativos de un manejo correcto del paciente
politraumatizado con sospecha de lesión medular cervical, en el contexto de la atención
prehospitalaria y de urgencias.
Método: la revisión narrativa ha sido realizada a través de la búsqueda bibliográfica en 5
bases de datos de ciencias de la salud (PUBMED, CUIDEN, SCIELO, CINAHL,
COCHRANE), y la biblioteca online ENFISPO, entre diciembre de 2016 y enero de
2017. Se utilizó lenguaje controlado y los operadores boléanos AND y OR. Y también
se realizaron varias búsquedas por lenguaje libre en la base de datos SCIELO.
Resultados: se seleccionaron 25 artículos, cuyos datos han sido clasificados en 4
categorías de resultado. Destacan las escalas ASIA, Canadian C-Spine Rule y NEXUS
como herramientas de valoración. En cuanto a técnicas y criterios de inmovilización, se
ha revisado la eficacia de varias de ellas en diferentes contextos.
Discusión y conclusiones: una técnica de inmovilización inadecuada está asociada a un
aumento de la mortalidad. Se hace necesario unificar criterios en cuanto a la valoración
e inmovilización, así como mejorar la formación de los profesionales que intervienen en
el tratamiento inicial de estos pacientes. La enfermería cobra especial importancia
durante la valoración y la reevaluación constante del estado del paciente. Se han
encontrado como principales limitaciones la controversia en cuanto a la clasificación de
los traumatismos, junto con el tamaño muestral escaso y aspectos éticos. Title: Initial management of multiple trauma patients with suspected acute cervical

Introduction: trauma form 12% of the global burden disease, and in the polytraumatized
patient, they are the most frequent at the cervical level (38%), and associated in the
most of cases with an acute spinal cord injury. It is estimated that 25% of existing
damages in patients with cord injury might happen or be increased during the
prehospital management. Therefore, the objective of this work is to analyze those
procedures which are indicative of a right management of the polytraumatized patient
with suspected cervical spinal cord injury, in the context of prehospital and emergency
attention.
Method: the narrative review has been carried out by the bibliographic search in 5
database of health sciences (PUBMED, CUIDEN, SCIELO, CINHAL, AND
COCHRANE) and in online library ENFISPO, between December 2016 and January
2017. For it was used controlled language and boolean operators AND and OR. In
addition, some search was performed about free language in SCIELO database.
Results: 25 articles were selected, whose data have been classified into 4 categories of
result. It highlights ASIA, Canadian C-Spine Rule and NEXUS scales whose valuation
tools. With regard to techniques and immobilization rules, the efficacy of several of
them has been reviewed in different contexts. According to the classification bruised
and penetrating trauma, the immobilization is not advised in the last one.
Discussion and conclusion: an inappropriate immobilization technique is associated
with an increase in mortality rates. It is necessary unify criterion about to valuation and
immobilization, as well as improve the training of professionals to intervene in the
initial treatment of these patients. The role of nursing is especially important during of
valuation and constant reevaluation about the patient’s condition. It has been found as
principal limitations the controversy about the trauma classification, in adition to
sample size and ethical aspects
