dc.description.abstractEl trastorno de déficit de atención e hiperactividad (TDAH) consiste en una alteración del neurodesarrollo, definido por niveles problemáticos de inatención, desorganización y/o hiperactividad-impulsividad que interfieren con el funcionamiento o desarrollo del niño. Este trastorno puede afectar, independientemente del entorno cultural, hasta a un 5% de los niños2, siendo su prevalencia el doble en el sexo masculino, para grupos de edades similares. OBJETIVO: El presente trabajo tiene como objetivo realizar, a través de una revisión narrativa, una evaluación de la seguridad y eficacia del tratamiento farmacológico en una patología altamente prevalente en el área de la salud mental infanto-juvenil: el trastorno de déficit de atención e hiperactividad (TDAH). MÉTODO: Se realizó una revisión narrativa mediante una búsqueda bibliográfica en la base de datos PubMed de los artículos publicados entre 1995 y 2017. RESULTADOS: A través del análisis de los artículos revisados, se identificaron los principales fármacos de uso en población pediátrica para el tratamiento de pacientes con TDAH: metilfenidato, atomoxetina, guanfacina y lisdexamfetamina. Según los resultados obtenidos, el más eficaz es la lisdexamfetamina, seguida del metilfenidato, y por último la atomoxetina. La guanfacina ha demostrado ser eficaz de forma coadyuvante junto a medicación psicoestimulante. Asimismo, la seguridad de los fármacos, de mayor a menor, es lisdexamfetamina, metilfenidato y atomoxetina. La seguridad de la guanfacina se evaluó únicamente frente a placebo. CONCLUSIONES: El tratamiento disponible para el TDAH en población pediátrica es seguro y eficaz en la mejora de los síntomas. Sin embargo, existe un gran desconocimiento sobre los beneficios y los efectos adversos a largo plazo de la terapia farmacológica utilizada en los pacientes con TDAH, por lo que es necesaria una mayor investigación para la mejora de las opciones de tratamiento. Por parte de enfermería, es imprescindible conocer el balance beneficio/riesgo de cada uno de los fármacos disponibles, para tener capacidad de asesoramiento a padres/cuidadores y pacientes.es_ES
dc.description.abstractThe attention-deficit–hyperactivity disorder (ADHD), consists of an impairment of neurodevelopment, defined by problematic levels of inattention, disorganization and/or hyperactivity-impulsivity that interfere with the functioning or development of the child. This disorder can affect, irrespective of the cultural environment, up to 5% of children,2 being twice the prevalence in males, for groups of similar ages. AIM: The present paperwork aims to conduct, through a narrative review, an evaluation of the safety and efficacy of pharmacological treatment in a highly prevalent pathology in the area of mental health of children and adolescents: attention deficit hyperactivity disorder (ADHD). METHOD: A narrative review was made through a bibliographic search in the PubMed database of articles published between 1995 and 2017. RESULTS: Through the analysis of the reviewed articles, we identified the main drugs used in the pediatric population for the treatment of patients with ADHD: methylphenidate, atomoxetine, guanfacine and lisdexamphetamine. According to the results obtained, the most effective is lisdexamphetamine, followed by methylphenidate, and finally atomoxetine. Guanfacine has been shown to be effective nearby psychostimulant medication. Likewise, the drug’s safety, from highest to lowest, is lisdexamphetamine, methylphenidate and atomoxetine. The safety of guanfacine was assessed only against placebo. CONCLUSIONS: The available treatments for ADHD pediatric patients are safe and effective in improving symptoms. However, there is a high uncertainty about the long-term benefits and harms of pharmacological therapy used in ADHD patients, so further research is needed to get new evidence for evaluating and improving treatment options. Is almost mandatory form the nurse perspective to know properly the risk/benefit balance for each of the drug currently used, to provide appropriate advice and guidance to parents/caregivers and patients.en_US
