Abstract
Introducción. El Trastorno por Déficit de Atención con Hiperactividad (TDAH) es de los trastornos mentales más frecuentes en la infancia, definido por sintomatología inatenta, impulsiva e hiperactiva; la prevalencia en España se estima en el 5-10% de la población infantil. Se caracteriza por etiología multifactorial, predominancia masculina y comorbilidad con otras patologías mentales. El tratamiento multimodal es la recomendación principal en el abordaje del trastorno e incluye entrenamiento parental.
El TDAH implica empeoramiento de la calidad de vida familiar, mayor hostilidad en los progenitores, baja autoestima, estrés y desesperanza. La disfunción del proceso familiar sugiere desarrollar programas de Educación para la Salud llevados a cabo por la enfermera.
Objetivo. Favorecer el funcionamiento familiar adecuado en padres de niños escolares con TDAH que lleven seguimiento en el Hospital General Universitario Gregorio Marañón o Centro de Salud Mental de Moratalaz, en Madrid.
Metodología. Se elaboró una intervención grupal de cuatro sesiones de entrenamiento parental en medio hospitalario, basado en la teoría del aprendizaje social. Se abordaron conocimientos sobre TDAH, técnicas en el manejo de conductas disruptivas (tiempo-fuera, extinción, economía de fichas), relajación y autocontrol basado en la respiración, estilos parentales y habilidades sociales de escucha activa y comunicación positiva.
Conclusiones. El entrenamiento parental podría ser una herramienta fundamental en el ámbito competencial enfermero de promoción de la salud y proporcionar a los padres un proceso de aprendizaje significativo en la atención integral del TDAH. Introduction. The Attention Deficit Disorder with Hyperactivity (ADHD) is one of the most common mental disorders in childhood and it is defined by inattentive, impulsive and hyperactivity symptomatology. The prevalence in Spain has been estimated between 5-10% of the child population. It is characterized by multifactorial etiology, masculine predominance and comorbidity with other mental pathologies. The multimodal treatment is the main recommendation among the treatments of ADHD, and it includes parental training.
ADHD implies deterioration in the quality of family life, higher hostility in progenitors, low self-esteem, stress and despair. The disfunction of the family process suggests developing health educational programmes carried out by nurses.
Objective. Favoring proper family functioning in parents of school children with ADHD whom are being followed up by the Hospital General Universitario Gregorio Marañón or the Mental Health Center of Moratalaz in Madrid.
Methodology. A 4-session group intervention of parental training was elaborated in a hospital environment based in the social learning theory. Knowledges about ADHD were taught, also techniques about the management of disruptive behaviors (time-out, extinction and token economy), relaxation and self-control technique based on breathing, parental styles and social abilities of active listening and positive communication.
Conclusions. Parental training might be an essential tool in the nurse’s competential environment of health promotion, and it might provide a significative process of learning to parents in the integral attention of ADHD.
